We’d like to hear from people who’ve been diagnosed with type 2 diabetes. Share your experiences of living with the medical condition

Nearly 3.5 million people in the UK are living with type 2 diabetes. Figures from from the British Heart foundation show that the number of adults in the UK with the disease has risen by 65% since 2005.

The medical condition develops most often in adults but is becoming increasingly common in children.

We would like to hear from people in the UK with type 2 diabetes. Tell us about your experience of living with and managing the disease. We’d also like to know about the quality of the advice and treatment you’ve received since being diagnosed. We will use some of your responses in our reporting.

You can share your experiences using the form below and if you you prefer, you can do so anonymously.

Chance to have your say fill in the form here: http://www.theguardian.com/society/2015/dec/24/living-with-type-2-diabetes-share-your-experiences?
